Immunization Information
Students must be current on all required immunizations.  Contact your school nurse or Sandoval County Public Health Office for any questions concerning required immunizations for school attendance.

The required immunizations are as follows:
DTP, DTaP, Td (Diphtheria/Tetanus/Pertussis) Four doses; at least one must be given on or after the child’s fourth birthday.  A three-dose series is sufficient if immunization starts at 7 years of age or later.  One dose of 3-dose primary series must be Tdap, preferably the first, followed by two doses of Td.
Td/Tdap (Tetanus/Diphtheria booster) 1 dose Tdap is required for entry into 7th grade.  All students in grades 7-12 require proof of 1 dose of Tdap received; one dose of Tdap is required regardless of ofwhen the last Td containing vaccine was given.
Polio  (OPV/IPV) Four doses; however, three doses are sufficient if the last dose is given on or after the 4th birthday.  For students in grades K-5,  the final dose is requiredafter the 4th birthday.  A minimum of four weeks between doses is required, with six months required between the last two doses. 
MMR (Measles/Mumps/ Rubella) Two doses, the first administered on or after the child’s first birthday.  The second should be administered on/after the child’s fourth birthday.   
Hepatitis B Three doses with the second dose administered a minimum of four weeks after the first dose, and the third dose administered at least 16 weeks after the first dose and eight weeks after the second dose.  If the child is receiving immunization for the first time and is 11-15 years old, two-dose Recombivax HB vaccine is valid if second dose is administered at least 16 weeks after the first dose.
Varicella (Chickenpox) 2 doses required for grades K - 7; second dose should be given on or after the child's fourth birthday. One dose required for grades 8 - 12; two doses recommended for 8th-12th grades.  A receipt of vaccine, physician/health care provider diagnosis of varicella, or laboratory evidence is required for proof of varicella immunity.
Additional Immunizations Required for Preschool Students Only:
Haemophilus Influenza type B (Hib) 3 doses if 1st dose is before 12 months, 2 doses if first dose is between 12-14 months,or 1 dose between 16 mo-5yr.
Hepatitis A One dose required by 18 months; two are recommended with at least six months between doses
Pneumococcal (PCV7/PCV 13) One dose PCV 13,24-59 months who are not completely vaccinated for their age. 3 doses by 12 months, 4 doses by age 15 months.  Children ages 14-59 months who have received an age-appropriate series of PCV 7 require an additional dose of PCV 13.
An annual influenza immunization is not required for school attendance; however, many physicians now recommend that children be immunized annually against flu viruses.  Parents may wish to discuss this suggestion with their family doctor or health clinic.

The New Mexico Department of Health suggests that fifth and sixth-grade girls be vaccinated against Human Papilloma Virus (HPV) to provide prevention against the virus that causes cervical cancer.  Parents may wish to discuss this suggestion with their family doctor or health clinic.  This vaccination is not required for school attendance.  Please contact your family physician, health clinic, or the school nurse at your child ’s school for more specific information about any of the required or suggested immunizations.
